Northwest Evaluations, LLC provides convenient, confidential telehealth evaluations conducted by a Washington-licensed mental health and addiction professional. Whether required for legal matters, workplace licensing, probation, or personal insight, we deliver objective clinical assessments with written reports returned in 24–48 business hours.
Our evaluation options
All evaluations include a comprehensive telehealth clinical interview, record review, and a formal written report delivered securely within 24–48 business hours of completion.
Substance use evaluation ($450)
A focused clinical assessment evaluating chemical dependency patterns, personal history, and risk factors, complete with tailored recommendations and written documentation.
Mental health evaluation ($650)
A comprehensive assessment examining psychological symptoms, mental health history, and functional well-being for legal, employment, or personal treatment planning needs.
Combined evaluation ($850)
An integrated mental health and substance use evaluation providing a thorough dual-perspective diagnostic summary and single detailed clinical report.
Purpose and scope of our assessments
Clients complete evaluations for legal or family-law matters, probation requirements, workplace or professional licensing mandates, treatment planning, or personal clarity. While we assist many voluntary and mandatory clients, we do not provide DUI/DWI assessments, forensic evaluations, child-custody evaluations, competency evaluations, custody recommendations, or expert witness testimony. Please verify all requirements with your attorney, court, or employer before scheduling.
Preparing for your appointment
To ensure a seamless session, please arrange a quiet, private space with no interruptions, a reliable phone or computer with high-speed internet, and a valid government photo identification for identity verification. Have any relevant court records, workplace paperwork, or previous medical documentation uploaded or on hand. Please remember that you must be physically located in Washington State at the time of your telehealth appointment.
The evaluation process
Getting your clinical report is straightforward and confidential. First, book your appointment, fill out your secure intake forms, and submit payment via Jane App. Next, attend your secure telehealth session from a private Washington location. Our evaluator reviews your information and submitted records to conduct your clinical evaluation. Finally, your completed written evaluation report is securely delivered within 24–48 business hours after the session.
